Meet and Greet is a great way for prospective parents and new patients to ask questions, see how we work, and meet one of our providers. We want to help you make the best decision for your family.

How Does My Child Become a Patient?

FOR NEWBORNS

We are excited and honored that you are choosing us for your child's care. It is always special for us to be invited by new parents to join in the care of a family member. For newborns, please call our office. It is important we get you in for your first appointment within 1-3 days of hospital discharge. Please complete the new patient forms  prior to the visit to help make the appointment as smooth as possible.


FOR NEW FAMILIES TRANSFERRING

Welcome to our practice! Please call our office at (808) 263-8822 prior to registering so we can gather the necessary information and better serve you.


After you have spoken with our office, you may begin the registration process. If you need assistance with the registration process at any point, please call our office.


Step 1 : Please complete all of our online new patient forms. Before signing the Patient-Provider Agreement Form, review our Patient-Provider Agreement, Financial Agreement, and Notice of Privacy Practices. You may print the forms and fax them to 808-261-6749 or email them to admin@reispediatrics.com.


Step 2: Obtain a copy of your child's immunizations and email the record to our office. We will need your child's immunization record prior to scheduling.


Step 3 : Appointment time! After we review the immunization record and create your child's chart, we will contact you to schedule the first appointment.
